What is the best diet for high triglycerides?

My triglyceride levels were 361 but with great cholesterol levels (146 total). The doctor mentioned nothing about my blood sugar but told me to lower my fatty food consumption, refined sugars and sweets as well as limit my carbohydrate intake to once or twice a week. So, what would be the best eating plan (along with excercise, of course) to use?
